Guide to Required Documents for Passport Renewal Applicants Abroad

Iranians applying for a new passport in Canada must complete their passport issuance and document delivery process, and receive their passports, after creating an electronic file (profile) on the Mikhak system, completing the passport issuance application form, uploading images of the required documents, submitting online to the Canada Bureau at the Ministry of Foreign Affairs, or visiting the Interests Section of the Islamic Republic of Iran in person (with the tracking code received from the system).

Important Notes

• If the original birth certificate/national identity card is in Iran, your relatives can take it to the Ministry of Foreign Affairs and request a certified copy to be sent to this representative office.
• Changing the place of residence in the passport (to the United States/Canada) is only possible with the applicant’s direct request and having its conditions.
• If requesting to change the place of residence, its fee must be added to the issuance fee.
• If the place of residence in the passport issued by this representative office is registered as “Islamic Republic of Iran,” you must obtain a permit to leave the country by going to the passport office of your city of residence upon your first entry into the country.
• Indicating a foreign place of residence in the passport is considered a permit for repeated departures from the country (except for eligible individuals).
• According to the Iranian Civil Law, the place of residence (indicated in the passport) of women and children under 18 years of age is subject to the place of residence (indicated in the passport) of the husband/father. If you need to change the place of residence in the passport (to the United States/Canada), a written request from the husband/father (consent letter) is required.
• If your birth certificate is not photo-bearing and you are over fifteen years old, you must apply to replace your birth certificate at the same time as you apply for passport renewal.

Required Documents for Passport Issuance/Renewal

• Printout of the “Passport Issuance” application receipt received from the Mikhak system (select “expiration” as the reason for the request).
• Form No. 001 (for in-person applications).
• Original passport.
• Original birth certificate.
• Original or copy of national identity card.
• Original or copy of valid residency document.
• Original or copy of Military service completion/exemption card (For men between 18 to 50 years old).
• 2 new photos (according to the photo guide).
• Service fee as Money Order according to the consular service fee schedule.

Required Documents for Passport Separation

• Printout of the “Passport” application receipt received from the Mikhak system (select “expiration” as the reason for the request).
• Form No. 001 (for in-person applications).
• Original Passport that you exited Iran with.
• Original birth certificate.
• Original or copy of national identity card.
• Original or copy of valid residency document.
• Original or copy of Military service completion/exemption card (For men between 18 to 50 years old).
• 2 new photos (according to the photo guide).
• Service fee as Money Order according to the consular service fee schedule.

Required Documents for Issuing a Lost Passport

• Printout of the “Passport” application receipt received from the Mikhak system (select “lost” as the reason for the request).
• Form No. 001 (for in-person applications).
• Police report.
• Copy of lost passport (if available).
• Original birth certificate.
• Original or copy of national identity card.
• Original or copy of valid residency document.
• Original or copy of Military service completion/exemption card (For men between 18 to 50 years old).
• 2 new photos (according to the photo guide).
• Service fee as Money Order according to the consular service fee schedule.

Required Documents for Confirmation of Power of Attorney/Signature Attestation

• Form No. 001 (for in-person applications).
• Original valid Iranian passport.
• Original power of attorney that has been signed and fingerprinted through a video interview.
• Service fee as Money Order according to the consular service fee schedule.

Note: The power of attorney fee is stated below the power of attorney (+ video interview fee + postage fee).

Required Documents for Issuing Birth Certificates for Persons Under 18

• Printout of the “Birth Certificate Issuance” application receipt received from the Mikhak system.
• Form No. 001 (for father only) (for in-person applications).
• Completed Form No. 420 (for in-person applications).
• Original valid birth certificates of parents.
• Original birth certificate of child.
• Copy of parents’ national identity cards.
• Copy of child’s foreign passport.
• 2 new photos (for under 15 years) (according to the photo guide).
• 6 new photos (according to the photo guide) and 2 copies of fingerprint cards (for individuals 15 to 18 years) (for in-person applications).
• Service fee as Money Order according to the consular service fee schedule.

Note: After issuing the child’s birth certificate, its image will be emailed to register the child’s passport application in the Mikhak system. As soon as you receive the birth certificate image, log in to the parent’s profile and register the application for issuing a passport for a child under 18 years. Note that the reason for the request is “without passport” in the “last exit border” section, and the date of the last exit from the border should not be written. It should be noted that the child’s birth certificate number is zero.

Required Documents for Issuing Birth Certificates for Persons Over 18

• Printout of the “Birth Certificate Issuance for Over 18 Years” application receipt received from the Mikhak system.
• Form No. 001 (for in-person applications).
• Completed Form No. 428 (for in-person applications).
• Original valid birth certificates of parents.
• Original birth certificate of applicant.
• Copy of parents’ national identity cards.
• Copy of child’s foreign passport.
• 2 new photos of parents (according to the photo guide).
• 2 new photos (according to the photo guide) and 2 copies of fingerprint cards of applicant (for in-person applications).
• Service fee as Money Order according to the consular service fee schedule.

Required Documents for Registering Marriage of an Iranian Man with an Iranian Woman

• Printout of the “Registration of Marriage of Iranian Man and Iranian Woman” application receipt received from the Mikhak system.
• Form No. 001 (for in-person applications).
• Completed Form No. 410 (for in-person applications).
• Original valid birth certificates and passports of man and woman.
• Official consent letter from the father for the daughter’s marriage.
• Original religious marriage certificate.
• Copy of national identity cards or passports of the officiant and witnesses.
• Original official marriage certificate.
• Service fee as Money Order according to the consular service fee schedule.

Required Documents for Registering Marriage of an Iranian Man with a Foreign Woman

• Printout of the “Registration of Marriage of Iranian Man and Foreign Woman” application receipt received from the Mikhak system.
• Receipt of “Issuance of Birth Certificate for Foreign Wife” application received from the Mikhak system.
• Form No. 001 (for in-person applications).
• Completed Form No. 411 (for in-person applications).
• Certificate of conversion to Islam for foreigners.
• Original valid birth certificate and passport of Iranian man.
• Official consent letter from the father for the daughter’s marriage.
• Original religious marriage certificate.
• Copy of national identity cards or passports of the officiant and witnesses.
• Original official marriage certificate.
• Service fee as Money Order according to the consular service fee schedule.

Note: After registering the marriage and issuing the birth certificate of the non-Iranian spouse, its image will be emailed to you to register the passport application in the Mikhak system. As soon as you receive the birth certificate image, enter the Mikhak system and create a profile for the foreign spouse with the provided national ID. Register the passport issuance application. The reason for the request should be “without passport” and nothing should be written in the “last exit border” section and the date of the last exit from the border. It should be noted that their birth certificate number is zero.

Required Documents for Registering Marriage of a Foreign Man with an Iranian Woman

• Printout of the “Registration of Marriage of Iranian Woman with Foreign Man” application receipt received from the Mikhak system.
• Form No. 001 (for in-person applications).
• Completed Form No. 412 (for in-person applications).
• Certificate of conversion to Islam for foreigners.
• Original valid birth certificate and passport of Iranian woman.
• Official consent letter from the father for the daughter’s marriage.
• Original religious marriage certificate.
• Copy of national identity cards or passports of the officiant and witnesses.
• Original official marriage certificate.
• Service fee as Money Order according to the consular service fee schedule.

Required Documents for Death Registration

• Printout of the “Death Registration” application receipt received from the Mikhak system.
• Form No. 001 (for in-person applications).
• Completed Form No. 440 (for in-person applications).
• Original passport of the applicant registering the death.
• Original death certificate.
• Original Iranian passport and birth certificate of the deceased.
• Original birth certificate of the deceased’s spouse.
• Service fee as Money Order according to the consular service fee schedule.

Required Documents for Divorce Registration

• Printout of the “Divorce Registration” application receipt received from the Mikhak system.
• Form No. 001 (for in-person applications).
• Completed Form No. 430 (for in-person applications).
• Original valid birth certificates of man and woman.
• Original valid passports of man and woman.
• Copy of national identity cards of man and woman.
• Original Islamic divorce certificate.
• Original or copy of local divorce certificate.
• Service fee as Money Order according to the consular service fee schedule.
